CAVEATS


Look closely at any private health insurance policy being offered by a trade association or other group, not all of them are legitimate, and you could end up with huge out-of-pocket expenses.

MANDATED BENEFITS

In addition to federally mandated benefits, each state has unique legislation that mandates insurance coverage for certain services and treatments.  Most group and private insurance plans (fully "self funded" plans and some specialty policies are generally excluded) are required to pay for these services regardless of whether or not your policy covers them.  Ideally your plan has already included these benefits in your policy, but they may not have, so it is important to know what they are.  Your first challenge is to find them, which isn't easy.  The information may not be available on the department website, so you will need to contact your state Department of Insurance for a list of the citations, and go to the Insurance Code for details.
